1. Client information for the client dossier

2. Process/procedure information for the process/procedure dossier

Client Dossier

Jeroen Wessels Hypnose maintains a client dossier for every (prospective) client. This dossier contains information such as:

- Data of the company/organization or location where business activities are actually performed, including: name and address details, company registration details, authorizations, powers of attorney, invoice information (only if the assignment is given by a company/organization).

- Data of the natural person, including: name and address details, ID information, date of birth, marital status, authorizations, powers of attorney, invoice information.

- Clients can manage data in the client dossier (online/not online) themselves. The client dossier is not public.

- If the assignment is terminated by the client or Jeroen Wessels Hypnose, or both, the client dossier is kept indefinitely unless the client requests removal.

Process/Procedure Dossier

Jeroen Wessels Hypnose maintains a process/procedure dossier for every executed process/procedure. This dossier contains all online/offline documents, medical and non-medical, related to the process/procedure.

- Data of the client(s), involved third parties, both medical and non-medical, both regular medical and alternative/complementary medical.

- Process/procedure dossier is not public, is confidential, and can be audited by competent authorities and regulators.

- Process/procedure dossier is retained indefinitely, even after the termination of the assignment, regardless of the reason for the termination.

- After the termination of the assignment, the client can request the removal of the process/procedure dossier.

Communication

Correspondence sent to us in any form may be stored and saved. Sometimes you may be asked to provide personal information related to the subject of the correspondence. This allows your correspondence to be treated correctly both procedurally and substantively. All information is stored on secure servers of Jeroen Wessels Hypnose or one of our contract partners. Jeroen Wessels Hypnose will not link this information to or use it in combination with other information.
